Service Description
During our first visit, we will learn about important factors that influence you and your health. We will discuss your health and nutrition history, lifestyle, preferences, past and present challenges, and personal goals. All of this will help us begin to develop the best strategies for you, based on reasonable changes and realistic expectations to promote permanent success.
Payment and Cancellation Policy
Patient's preferred payment method must be provided at time of scheduling, and will be retained for scheduling all future visits. Any balance due will be charged at the conclusion of each visit. Patients choosing to utilize health insurance coverage are responsible for verification of benefits, and will be charged for any remaining balance due (deductible, copayment, or coinsurance) after claims have been processed. No-show or canceled appointments with less than 48 hours notice will be subject to a no-show fee of $50.00, which will be charged to the payment method on file. Failure to provide prompt payment for services rendered will result in termination of care.
