Service Description
There are many factors that influence the frequency and duration of our visits. During our initial consultation we will decide together how often we will continue to meet. As your needs and goals change, our follow-up plan will be adjusted to match.
Payment and Cancellation Policy
Patient's preferred payment method must be provided at time of scheduling, and will be retained for scheduling all future visits. Any balance due will be charged at the conclusion of each visit. Patients choosing to utilize health insurance coverage are responsible for verification of benefits, and will be charged for any remaining balance due (deductible, copayment, or coinsurance) after claims have been processed. No-show or canceled appointments with less than 48 hours notice will be subject to a no-show fee of $50.00, which will be charged to the payment method on file. Failure to provide prompt payment for services rendered will result in termination of care.
